A serious diagnosis. A debilitating injury. A sense that something is wrong, but your health care provider is not hearing your concerns. When you or a covered dependent are dealing with a complex medical issue, you may feel overwhelmed or lost at sea. You are not alone.

MESSA’s Medical Case Management program is here to help. MESSA has a team of nurse coordinators who are registered nurses and board-certified in case management. Through this program, they work directly with members and their families to identify specialists, facilities, resources and services. Their unique blend of medical expertise and case management skills provides support, advocacy and peace of mind when you need it most.

Medical Case Management program is a free benefit for MESSA members and covered dependents, available at absolutely no cost.

Your MCM team

Members who enroll in the MCM program work with a MESSA nurse coordinator, as well as an external nurse case manager in their local community.

Together, this team assists you in navigating the health care system, identifying local resources, accessing benefits, and understanding your diagnosis and various treatment options.

MESSA’s Medical Case Management program has helped members by providing resources for:

  • Modifications for accessibility
  • Specialty durable medical equipment, such as hospital beds or motorized wheelchairs
  • Hospice support

Getting started

MESSA’s Medical Case Management nurses work with members of all ages who have had a variety of illnesses or catastrophic injuries, including cancer, cerebral palsy, multiple sclerosis, head or spinal cord injuries, amputations, severe burns, stroke and more.

What to expect

Once you’re enrolled, an external nurse case manager from your local area will be assigned to you and schedule an initial visit.

The nurse case manager will gather information from you and discuss your plan of care, including your individual goals and preferences. Throughout your MCM enrollment, the nurse case manager and the MESSA nurse coordinator will collaborate to support you in achieving the goals in your care plan.

The flexibility of the program allows your case manager to increase or decrease their involvement as your needs change and goals are met. The ultimate goal is to provide enough support and resources to help you regain your independence and feel confident in making your own treatment decisions.
